Enrollment Process

An application must be completed in order to be considered for enrollment. There are no exceptions. 
The application for Kindergarten - 5th Grade is available through the link above. Please be aware that the following documents will be required:
• Florida Certification of Immunization or Exemption• Physical examination (must be no more than 12 months old at the time of the student’s start date)• Copy of Parent/Guardian’s driver’s license or state issued photo ID• Copy of Birth Certificate• Copy of Social Security card• Copy of most recent report card• If applicable, copy of standardized test scores• If applicable, most recent IEP or 504• Proof of Address
For proof of address, you can send two items from Group A or one item from Group A and two from Group B. 
Group A:• Jacksonville Electric Authority (JEA) bill or other utility or cable/internet bill• Mortgage Agreement (or closing statement)• Lease Agreement (from a company, or *private individual)• Driver’s License or state issued IDGroup B:• Automobile Insurance• Credit Card Statement• Bank Account Statement• US Postal Service Confirmation of Address Change• Current Bank Account Statement (may block out account number)• Payroll Statement• Current Non-cellular Phone Bill• Vehicle Registration*Private lease agreement must be notarized AND two additional documents from columns B are required.
Parents will also be required to register for a Duval County account. The office will provide you the information on how to do that once your enrollment has been accepted. 
If you require assistance with this process, contact the school office between the hours of 8:00 am – 2:00 pm Monday – Friday (Summer Hours: Monday – Friday from 9:00 am – 3:00 pm). Please note, during the school year, the office is closed for dismissal from 2:30 pm - 3:20 pm daily.

Controlled Open Enrollment

Controlled Open Enrollment allows a parent from any school district in the state to enroll his or her child in and transport his or her child to any public school, including charter schools, that has not reached capacity.
